First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. At your consultation, you will receive a detailed treatment plan outlining cost, estimated insurance and your portion due the morning of surgery. After which, you will be able to schedule your surgery appointment.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip.
  • A list of medications or injections you are presently taking.
  • If you have medical or dental insurance, bring your insurance cards or information with you. This will save time and allow us to help you process any claims.
  • Go to the patient registration tab to print the patient information and health history forms.  Bring these completed forms with you to your consultation visit.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.). Also it is important for you to notify us if you take medication or have injections for bone density.
